REQUIREMENTS FOR DELAYED REGISTRATION OF BIRTH

1. Negative Certification of Birth from PSA (formerly NSO)

2. Certificate of Live Birth (COLB) [four (4) copies)- duly accomplished - Affidavit of Acknowledgement/Admission of Paternity (dorsal portion of the COLB) Must be signed by the mother if parents are not married, father may sign the affidavit of admission of paternity if he wishes - Affidavit for Delayed Registration of Birth (dorsal portion of the COLB) Must be signed by the mother/father/guardian or other persons allowed by law

3. Certificate of Marriage (if possible, No. 4 is not required, skip to No. 5)- skip to No. 4 if parents are not married

4. Affidavit to Use the Surname of the Father (AUSF)-if parents are not married and surname of the father shall be used, father must acknowledged the child by signing the Affidavit of Acknowledgement/Admission of Paternity at the dorsal portion of the COLB or in other legal instruments, mother shall execute the AUSF if registrant is a minor or the registrant himself/herself if of age

5. At least two (2) of the following:

a) Baptismal Certificate

b) Form 137 or School Records showing facts of birth

c) Medical Record (Immunization Card/Hospital Certificate) showing facts of birth

d) Income Tax Return of registrant or registrants parent/s (if minor)

e) Voter's Affidavit or ID

f) Certificate of Marriage (if registrant is married)

g) NBI Clearance

h) Insurance Policy

i)SSS (Form E1 GSIS of registrant or registrants parent/s (if minor)

j) Any public Document or ID of the registrant showing facts of birth

6. Affidavit of Two (2) Disinterested Persons

7. Affidavit of Delayed Registration 8. Processing fee (Php200.00)
